The Case Management Department provides multiple services at Blue Peaks. It is the entry point for services and supports. Case Management staff determine eligibility for services, following criteria set by the State of Colorado and Medicaid. When a person meets the criteria, they may be enrolled or placed on a waiting list for services (not all programs have a waiting list). The Case Manager coordinates services and supports through the development of a Service Plan. Service planning is done in a person-centered fashion and includes the person’s individual goals and desires. The Case Manager revises and updates the plan as needed to meet changing circumstances. The Case Management staff at Blue Peaks are always available for consultation and referral regardless if a person is enrolled in services.
